No. Dr. Jacobs is out-of-network with all insurance providers including Medicare. This means that you must pay for your appointment at the time of service. Upon request, we can provide a superbill, which explains the charges, which you can submit to your insurance. Depending on your specific plan, you may be eligible for some reimbursement, especially if you have out-of-network benefits.

Yes. Dr. Jacobs has “opted-out” of Medicare. This means she can see patients with Medicare and Medicare Advantage plans with the same fee structure as other patients. You will be required to sign a private-pay medical services contract to indicate that you understand that you are paying directly for services and that you will not be able to seek reimbursement from Medicare.

New patient visits (in office or telehealth): $500 (up to 1 hour)
Follow-up visits (in office or telehealth): $300 (up to 30 minutes). Additional time will be billed at $100 per 15-minute increment.
There is no membership or subscription fee.
If you cancel your appointment with less than 24 business hours’ notice, or if you do not show up for your appointment, you will be billed for the full appointment fee.

While we do not provide on-call or after-hour services, you can send a message through the secure patient portal for non-urgent questions. Messages will generally be answered within 2 business days. If you are having a medical emergency, you should call 911 or go to urgent care or an emergency room. If you have sleep equipment problems, you should reach out to the equipment supplier.
If you need help with home sleep testing, please call the device supplier.

For PAP supplies to be covered by your medical insurance, you need to have an active prescription. If you have not seen Dr. Jacobs in this practice before, you will need a new patient appointment to establish care at which time a prescription for supplies may be written. If needed urgently, PAP supplies can be purchased out-of-pocket online and locally at DME (durable medical equipment) companies and some drugstores.

If medications are appropriate for your condition, Dr. Jacobs will prescribe treatment.
Many prescription medications for sleep disorders are controlled substances which will require periodic follow-up appointments, including at least one in-person visit. Refill requests should be made through the pharmacy a week before the medication is needed.
