Patient Journey

This outline will take you through the steps of my evaluation and treatment process.

1.

Book an appointment!

Request a date and time for a new patient appointment from the calendar. We will contact you typically within 2 business days to confirm your request. You will then be sent a link to complete the intake forms, which should be submitted at least 2 days before your appointment.

2.

Complete intake forms

These forms will include medical information, both current and past, as well as administrative forms that are required before you can be seen. If the forms are not received at least 2 business days in advance, your appointment may need to be rescheduled. You can upload prior sleep records or anything else you think will be helpful for your evaluation.

3.

Sleep Consultation

Your story matters and takes time to tell. We will discuss your goals for evaluation. We will determine if you need sleep testing and if it can be done at home.

4.

Sleep Testing

Your test will be ordered at an affiliated facility. Home testing can often be done within 2 weeks; laboratory testing takes a bit longer.

5.

Review Study Results

At this appointment, we will review the test results, how they may explain your complaints, and risks to your health. We will determine which treatment option makes the most sense for you.

6.

Treatment

Treatment will be ordered through another provider and mostly managed by Dr. Jacobs. PAP is ordered from a DME (durable medical equipment) provider, oral appliances from a Board-Certified Sleep Dentist, and other interventions from specific providers. If medication is appropriate, Dr. Jacobs will write prescriptions.
